Please note: Key fobs/access cards issued before July 2025 will not work with our new security system. To ensure continued building access, please complete the renewal form below.

Application Form (New Users)

If you do not have a Chapel key fob/access card/mobile pass, please complete this form and select “New application” for Application Type. A one-time fee of $20 will be charged for non-Chapel members.

Note: If you’ve already submitted an application for a key fob/access card/mobile pass but have NOT paid the $20 fee, you can proceed directly to our payment page.

Renewal Form (Current Users)

If you already have a Chapel key fob or access card from a previous semester, please complete this form and select “Renewal” for Application Type. You will be given the option to choose between a new key fob or access card, or a mobile pass.

No fee is charged for renewals; however, we do appreciate your donation to help defray costs. A link to the donation page will be shown after you complete your renewal.
